Change Healthcare Letter Template

On February 21, 2024, our practice and many others that use EPIC software were subjected to a cyber attack. This resulted in claims not being processed or received by the payor.

Please attach proof of timeliness to this letter along with a printout of the claim when submitting the appeal/dispute.
