Steps CCS Eligibility

  1. When verifying Medi-Cal and the patient indicates CCS, it is important to recognize that we should bill CCS as the primary payer. The secondary payer would be the HPSJ in this scenario, or any other payer indicated on the patient's Medi-Cal eligibility.

  1. Once this page populates you wil Enter either the CIN (aka Medi-cal ID number) or first name, last name, and DOB.

Click on the patient


  • All the (SARs) will be listed below along with the dates. Please make sure to use the SAR that includes our providers. The specific SARs that are applicable for our practice are PHYSICIAN & CARDIAC CENTER
  • SAR NUMBER is used for the patient authorization on the superbill. Please make sure the dates are current.

If a patient does not have an established SARS with our office and they are coming for these diagnosis we must apply for CCS. Listed below are the CCS Diagnosis.


  • Q21.2 -AV CANAL
  • Q25.1 COA (COARCTATION OF AORTA)
  • Q22.5 EBSTEIN ANOMALY
  • Q23.4 HLHS
  • Q25 INTERRUPTION OF AORTIC ARCH
  • Q21.3 TOF ( TETRALOGY OF FALLOT)
  • Q20.3 TGA

If a patient has CCS profile in CCS please copy and paste all the information to the form examples case number - CIN -Alias etc....


Below is an image of how to file out the form for our practice.


Last step would be to fax the CCS county that the patient belongs in. Please fax the medical report and referral from the PCP for New patients for establish patient just the medical report.


Please save the copy of the fax to the P-Drive for reference.
