Prime Community Authorization


Click on Auth/Referral Submission




When entering the member ID, please input the member ID and click on "Search".


Authorizations will be listed below if an authorization has been requested.


  1. Add Member ID to the search Bar ( if the member does not populate please reach out to Prime community care to get them added to there system. 1-855-343-2247)



  1. Next is to add our NPI

click on our TAX ID

After this page has finished populating, please click on the Search button.


Select the clinic that is relevant to our provider and clinic.


Click on the same as requesting Provider.


Add the diagnosis that correlates to the authorization


Kindly include the codes that you are requesting. I would like to request all the codes to ensure that the Medical Assistant has all necessary information available for the Physician if needed.


Please type in the reason of our request.



Lastly, the next step is to upload the required documents: referral and/or medical report.

Please click on the print and save the document in the P-Drive for future reference.
